- A page titled “PowerForm Signer Information” will pop up. Please fill in your full name (first and last name), and valid email address. An access code will be sent immediately to the email address you indicate; the email will be from “Christine Kembikian.”
- Enter the access code into the form on your browser to access a personalized, secure consent form. If you have closed your browser, you may click the “Resume” button in your access code email. Enter the access code, and then click the yellow button “Validate.”
- You will see the consent form. At the top of the page, please read the “Electronic Records and Signature Disclosure.” Check “I agree to use Electronic Records and Signatures,” and click the yellow button “Continue.” (Clicking this does not obligate you to participate in the study.)

- Fill in the form by clicking your responses to the required “Yes-No” questions. At the bottom, click the yellow “Sign” button to electronically sign your name on the line.
- Confirm that your full name and initials are correctly listed. Edit if anything is incorrectly listed. If correct, click “Adopt and Sign” to complete the form.
- You will receive a confirmation email, with access to a copy of your signed consent form. You will be able to view and download your completed consent form.
